home. The purpose is to provide necessary care while teaching the
patient/caregiver how to safely manage the care on their own.
To qualify for home health benefits you must be:
Homebound
Under the care of a licensed physician.
In need of one of the following skilled services:
-Intermittent skilled nursing care
-Physical Therapy
-Speech Therapy
Once the requirements are met, additional services may be covered including Home Health Aide, Occupational Therepist, and Medical Social Worker.
These services must be ordered by a physician and must be considered by your insurance to be reasonable and necessary for the treatment of your illness or injury.
Home Health Services are intermittent with the availability of support by on-call nursing staff 24 hours a day, seven days a week, 365 days a year.

Q Can Assured Home Health help with a patient who lives alone, has been falling, and seems a little confused?
A A referral may be made to the Intake Department. An intake nurse will establish homebound status, coverage for services, and contact the physician for initial orders. A Nurse or Therapist will visit to evaluate what the needs are. A plan of care is then developed and sent to the physician for approval.
Q What do you mean by "homebound"?
A Homebound means it requires a taxing effort for a person to leave home. In many cases the patient only leaves the home for approval.
Q Who pays for Home Health Services?
A Home Health Services are benefits under the Medicare and Medicaid programs. Many insurances cover the home health benefit also.
Q How do I find out if a patient is covered under home health services?
A Assured Intake nurses will make the necessary calls to determine insurance coverage for services. they will notify the patient and obtain physician orders as necessary.
Q What kind of training does the staff have?
A All staff have current licenses specific to their discipline. Education is offered to all staff within the agency to ensure adequate knowledge and current standards of practice.
Q What are the goals of home health services?
A Goals are established as part of the plan of care determined by the home health staff together with the patient and/or family. The primary goal is to help a patient toward recovery while remaining independent in their own home.
