What is Home TeleCare?
Home Telecare is a method of providing care, monitoring and education to our patients at home using telecommunications technologies. The program uses live audio and video and medical peripherals that can be used to monitor blood pressure, check weight, assess blood oxygen levels, or assess blood glucose levels. The units available have options to use the peripherals or units that provide only audio or visual communication with a nurse for teaching and assessment.
What is reason for this type of service?
Home TeleCare Services are used to provide additional or supplemental visits to the in-home care provided by the Home Health Staff. They are conducted by a skilled clinician and are usually done between onsite visits or if problems arise, but the visit doesn't require hands on care.
Why is this service beneficial?
Advantages to our patients include: frequent contact by the clinician, elimination of the travel time that is needed to see a patient in person, comprehensive health education and monitoring of patients status, and more continuity of care.
How is this service provided?
The Home TeleCare visit is provided to the patients in their home over a telephone line. The patient will have a small unit in their home that has a camera and phone attached. The clinician initiates the video visit from the Central Station located at the Home Health Office and communicates visually, audibly, and through the peripherals with the patient at home. The unit in your home cannot initiate the visit and the unit is never used to monitor your activities without your knowledge.
What type of care can be provided with these visits?
Care that can be provided via Home Telecare includes: Viewing of patients; Assessment of patient condition; observation of medication administration including oral or injections; monitoring of vital signs; checking weight; assessing mental, functional, and overall health status, and specific tests, depending upon the unit's specific capabilities.

The patient, caregiver, and clinician are “virtually” put in the same room.

The AVIVA Home Telecare System consists of the Central Station, which connects via ordinary telephone lines to the Patient Station, placed in the patient's place of residence.
